To book a session, please complete this form by providing your information and reason for your visit. You can also include your preferred Psychologist, if known. Once your booking is complete, you’ll receive a confirmation email from our team with all the details you need for your appointment.

Please note that for services involving a minor, consent from all parents or legal guardians with decision-making authority is required before services can begin. If there are specific custody or decision-making arrangements, documentation (such as a court order or custody agreement) may be requested.
